I think you mean 5 nursing diagnosis for the above condition, not 5 care plans, correct?

A good care plan book is invaluable...if you don't have one yet, I'd recommend getting one, ASAP.

see this thread on how to construct a nursing care plan: https://allnurses.com/general-nursing-student/help-care-plans-286986.html - help with care plans. the 5 nursing care plans you are talking about are actually 5 nursing diagnoses, or nursing problems, for someone who has a medical diagnosis of hypertensive cardiovascular disease. you must first break down the hypertensive cardiovascular disease into it signs and symptoms as well as consider its causes and risk factors. the signs and symptoms can be used as the evidence of nursing problems. every nursing problem (nursing diagnosis) has a defined set of symptoms called defining characteristics. we nurses treat the defining characteristics--they become your plan of care because you are planning to do something about that nursing problem. start by learning as much as you can about hypertensive cardiovascular disease:

for example, obesity and smoking are risk factors of hypertension. if someone with hypertensive cardiovascular disease is obese you might want to give them a nursing diagnosis of imbalanced nutrition: more than body requirements because they eat more food than is necessary. the nursing interventions for your care plan would include things that would involve a weight loss plan.
