i heard rumors in multiple choice that you can not choice your answer such as "call the physician immediately." as one of the 4 choices in nclex.. is this true or not? can you clarify this matter and i really appreciated. and thank you.

"Call the physician immediately" is usually not the correct answer to an NCLEX question. There is usually a nursing intervention that can be performed first (elevate the head of the bed, administer prescribed analgesics, etc).
