I don't have health insurance right now, but I have all the immunization shot done before, do I need find a doctor to fill out and sign his name on the health status report sheet for apply CPNE, or just sent out the proof of immunization record to excelsior ?
If I have to find a doctor, where to find one without ask my health insurance? I am in Houston area.
I am so frustrate with this piece of paper. ( performance examination health status report)

I'm pretty sure it has to be signed by a doctor, sorry I know that's not what you wanted to hear.

Call Excelsior. I believe it needs a doctor's dignature but ask about the immunization form.

To save some money, you could go to a nurse practitioner. I did my paperwork, including my separate disability stuff (for a latex allergy, for crying in the corn) with my nurse practitioner. Everything was accepted.
I called a Nurse practitioner office , she will charge me 50 bucks for
General Office Visit, for fill out health status report, is that sounds good?

Cripes--that's better than my NP charges, and I'm in podunk cheapo Iowa. I say it sounds good! I just made sure to have all my little ducks in a row (my immunization records) right there in my hot little hands with the dates so it was an easy job for my NP to put them in the impossibly tiny spaces to write in.
