    If you log in to evolve and click to see your detail score it will tell you what you did wrong. I believe you first- view detail score and then on your right -view detail report, something like that....

    You don't need to fill out an Application, I went to the Nursing Department and all I did was turn in a copy of my Hesi Score along with my student Id, then I paid for the application at the Cashier...
