    Honestly, it is because of this site why I could complete all my processing on time..I'm just waiting on my eligibility so I could take my test asap and start working as an RN! Hehe

    If you have a valid license in any American state, yes! If not, NO I assume you're in Phil applying fresh, so NO. Also, form 3 isn't needed!
