Has anyone gotten the health form to be completed ? I got it a couple of weeks ago and it said in the letter they will email us the website where we have to upload the completed health form but I haven't gotten any email as of yet !

Yeah I am starting in Spring 2016. when i paid the online deposit, i did fill out some information that they asked and that was it. i will shoot an email to the admissions office and see what they say. thanks for your help
