pressure ulcer coding

When a stage 3 pressure ulcer is healing well and at the time of the MDS presents as a definite stage 2, (but being coded by nursing as a "healing stage 3" ,do I code it on the MDS as a stage 2 or a stage 3?

I don't know why all of a sudden I am questioning this!:woot:

I have read and re-read the RAI manual and it speaks about the clinical aspect of coding areas as "healing stage" whatevers but I can't seem to find where it specifically says to code the MDS according to it's physical presentation or by it's clinical dx.

Here's what it says in M-6

"If the pressure ulcer has ever been classified at a higher numerical stage than what is observed now, it should continue to be classified at the higher numerical stage."

It stays a stage 3 until healed or ever becomes unstagable or stage 4.
