Postcard 55A-7 for LVN application, where can I get this?

Hi,

This may sound like a dumb question but pls bare with me as I am new to the LVN process.

My question is, where can I get a postcard (55A-7). It says on the instruction to "Write your name and address on the post card provided. Make sure to place a postage stamp on the postcard to receive verification that your application was received by Board. ( Note: Not applicable for applications downloaded from the internet)

FYI: i have downloaded my application form from the net. Does this mean, I dont need to have this post card?

another question is, did you send your application thru UPS?

Yes you don't need it if you just downloaded your application from the Internet.

Though what i did to get the verification that they received my application was to send them a postcard bought from a dollar store, with my name and address and a post stamp,submitted together with my application. And after 2 days i got my post card back with the bvnpt stamp on it.
