I am still getting used to writing careplans and I need a respiratory careplan for my post-op pt. I think it should be Ineffective breathing pattern but I want to incorporate the surgery and his hx of sleep apnea together. Any ideas on how to word the diagnosis?

I used a care plan book throughout LPN school for developing care plans. It breaks down by systems as well as has sections on post-op patients. Care Plan books are a great tool and will really help you in developing good care plans.
Remember to prioritize by ABCs and Maslow's hierarchy of needs.

I just had one.. My guy had tongue cancer.. For his priority nursing diagnosis i had...
Ineffective Airway Clearence related to laryngeal edema as evidence by dyspnea.
What type of surgery did he have?? Maybe Ineffective breathing pattern related to post op surgical incision as evidence by s?? (i believe you can use sleep apnea bc that is a medical diagnosis not a nursing diagnosis.. but im not sure)
Hopes this helps somewhat but im only a first year nursing student and its all new to me as well
