When filling out a school physical exam form for a male patient, do you include whether they' are circumcised or not in the genitalia section?

Unless there is a medical concern or disease process involved that would impede activity I can not think of any reason you would include that.

Is there a spot on the school form to indicate? If not, then don't chart more than you have to.

On school/work PE forms, I fill out what's asked and no more. Of the occupations/schools a) that require a PE form be filled out, and b) where a person's circumcision status matters, I don't think the Venn diagram circles overlap too much.

On our EMR, there is a spot for intact foreskin vs. circumcised. I chart there, just like I'd chart if I saw Mongolian spots on a baby's butt. It's an observation, nothing more.

Even performing wellness checks in my NP clinical rotations we did not examine the member on a child over maybe 2 years old unless the parent pointed out a problem. I cannot imagine any reason a school would need to know if a make was circumcised or not.

Also consider neither condition is considered "abnormal" in this country. Neither technically is a condition that you could treat or has a risk specifically related to schools or personal well-being.
