Open Heart ICU, Student Precepting

Hello!

I'm a senior nursing student and just found out that I have received a one-on-one preceptorship in an Open Heart ICU, also referred to as the Cardiovascular Recovery Room. In recognition of the acuity and specialty of this unit, I want to prepare as much as possible before stepping onto the unit.

As cardiac nurses, what would you recommend I review? I'm really excited by this opportunity, and want to hit the ground running (and avoid "being in the way").

I'm planning on reviewing CVP monitoring, PAWP, and ventilators to start and would appreciate any advice for further preparation.

Unit Description

"The majority of the patients have undergone surgery for coronary artery bypass grafts (CABG) and valve repair/replacement. Advanced therapies include LVADs, , IABP, CRRT , ECMO and Arctic Sun."

Might want to brush up on your cardiac rhythms- it's not uncommon to see pacing and/or arrhythmias after open heart surgery.

I would also review your vasopressors and inotropes. Most people will come out on a dose of something-commonly epi. I would review those! Good luck!

Start getting comfortable with your rhythms, know how your SWAN works and the numbers that go with it, also learn how to manage a chest tube drainage system (Dos and Donts). The rest will come with your actual hands on experience.

Since you are still very new to critical care, this video I just finished on ICU Nurse's role during endotracheal intubation might help. You will see and be a part of many a intubation and knowing what to do and how to prepare for it is 3/4 of the battle.
