need help subjective data on a stroke patient!

doing a care plan on a CVA (stroke) patient and for my subjective assessment data I need lot of information so I can do my nursing diagnosis. Patient only answers to "yes or no" questions. and my instructor told me not to rely only on my objective data that I need subjective data. please help, how can I put subjective data together regarding this situation?:bugeyes:

Do you still have access to the patient? If so, I would make a list of common complaints of stroke patients (look in your textbook or online or something) and then ask those things as yes or no questions. For example, ask do you have a headache? Are you having difficulty swallowing? Do you have weakness? Is it on your right side? In your arm? In your leg?

If you do not still have access to the patient I would ask your teacher if you can add the things that are common to stroke patients in combination with what you think the patient had based on your objective assessment and your minimal subjective interview. I know that for my professors, we sometimes get so busy that we are not able to complete every little thing on our 18 page care plans, so they have us put in the expected response - as long as it makes sense for this individual patient.
