Nursing DX for postpartum couplet

I am working on a care plan for a couplet on the Mother-Baby unit. It was a text book perfect labor and delivery- G1P1, no IV, Mom delivered at 10:30 at night and was up and showered the next day. She is breastfeeding without complications, and baby is healthy. Mother is married with a supportive husband. What are some good nursing diagnoses for this situation? Thanks!

You could use the "at risk for" dx. She is at risk for lack of sleep, fatigue, dehydration, imbalanced nutrition ( less or more that body allows) pain, ( r/t recent lady partsl birth, new breast feeding mom) PPD ( postpartum depression) The " at risk for " nursing dx are always good, because you can use your interventions that teach to prevent and notice the s/s of impending issues. Lots of good interventions with these. Let me know if you need more clarification or help. New moms are always at more risk for these things also.

  • effective breastfeeding

for the baby:

  • effective breastfeeding
  • ineffective thermoregulation r/t immature compensation for changes in environmental temperature
  • risk for infection r/t break in skin integrity at umbilical cord site

For mom:

Since she is a first time mom you could go a psychosocial route such as:

Caregiver Role Strain r/t care of newborn

Social Isolation r/t care of newborn

or

Sleep Deprivation r/t breastfeeding newborn

Fatigue r/t care of newborn

Risk of constipation r/t narcotic pain medications

Impaired comfort r/t lady partsl laceration

Risk for deficient fluid volume r/t postpartum bleeding

Deficient knowlege r/t birth of newborn

Readiness for enhanced knowledge care of newborn

Acute Pain r/t lady partsl laceration

Impaired tissue integrity r/t lady partsl laceration

Risk for infection r/t lady partsl birth

For infant:

Risk for imbalanced body temperature r/t immature thermoregulation

Risk for imbalanced nutrition: less than body requirements r/t breastfeeding

Impaired Tissue integrity r/t removal of foreskin (if this is a male baby being circumcised)

Risk for ineffective infant feeding patterns r/t breastfeeding
