Nursing Diagnosis Question

Published

I have a patient with multiinfarct dementia.

One of my diagnoses is Chronic Confusion.

It's the R/T part that I am questioning......according to my Nursing Diagnosis Handbook one of the R/T's listed IS multiinfarct dementia.....but I can't write that because that's a medical diagnosis, right?

Can anyone help me write it correctly? Thanks!

how about r/t neurological dysfunction

Thank you Thank you!

Sometimes I just can't put what I am thinking into correct terminology- but you helped a ton!

Another possibility is "Chronic Confusion r/t organic brain dysfunction secondary to multi-infarct dementia, AEB...." - In our program, we allow medical diagnoses in the "secondary" part of the etiology. Check with your professors/ instructors as to what is "kosher" in your program.
