    Hi Karen,
    I live in Tennessee. We have mandatory reporting of unusual events. Very well defined guidelines by the state. I work in Risk Management as their clinical specialist. I'm an RN.

    I'm glad to see that the ANA is supporting voluntary reporting. It's the only way that we can "fix" our systems that are flawed. We have a long way to go to change cultures but I think we're on the right track. Starting with a non-punitive environment. I know that the reporting has been a positive step for our facility.
