Nurse's role following implementation in nursing care plan

Hi, I need some help... I am just learning about the nursing process and have read some great posts on here... Im just wondering if anyone could help me identify the nurses role following implementation and describe how this leads to a positive outcome for the client. I just need direction please. Thanks. I am a year one nursing student in New Zealand.:)

you've implemented the care plan (initiated the care plan). the next step is to determine if the goals/outcomes you developed have been met. this is called evaluation. it is another form of assessment except you are specifically looking to see if your interventions worked to improve, stabilize or support the deterioration of the patient's nursing problems.

  • are the interventions working?
    • yes - continue the care plan as written
    • no - consider other interventions and use them in place of the interventions that were not working

    [*]have the interventions worked?

    • yes - the problem is solved and can be removed from the problem list
