I feel like a idiot, I saw a pt on Thursday dx strep throat ordered amox did not notice child was allergic to PCN , came back in on Friday with a rash changed the abx to azithromycin, pt back today not any better parents upset because child is not better , I feel like a failure

a good way to prevent this is to write on all your scripts the allergies on top or if none i write nkda and if they have allergies i double check. i also write the weight of kiddos on my scripts so pharmacy can double check if needed.

Thanks did not think of that will do that in the future but I am so distraught over it , even though no harm was done I am angry at myself over this

You learn from these mistakes. Instead of getting down, use it as a learning opportunity, and let yourself grow from it. :)

That being said, from now on record in a special spot any drug allergies, so you can easily view it.

Practicing medicine means your mistakes can have dire consequences, including killing someone. Unfortunately, those who practice medicine are mere humans, which means they are imperfect and will make mistakes.

Offer your profound apologies to the child/family (if allowed in your state), accept the administrative consequences, look for systemic changes which could prevent this mistake from happening again, and, most importantly, learn from your mistake so you are less likely to make this mistake again.

A formal complaint?! Has the doc NEVER done the same thing?

Also, just have to say that this obviously fell through several safety check points. I mean the parents didn't say anything? The pharmacy didn't ask about allergies when they filled it?

We have computerized charts and all Rx are either printed or eprescribed to the pharmacy. The computer pops up an ALLERGY alert if you try to prescribe something listed as an allergy in the chart. Also, I do make it a habit of asking just before I prescibe, even though I checked the chart, if the person is allergic to anything. Sometimes, it's not listed so I always like to check.

I doubt the complaint will go anywhere...

I know you're looking for support, but I have to say that this disturbs me a little bit. I would consider a penicillin rash "harm", these rashes can be very resilient and may ever require a patient to take a short course of steroids (which comes with it's own set of complications) to help overcome. I honestly don't mean to be blunt, but this type of thing should be easily avoidable in a clinic environment.

If this is the case, I would consider talking to your or some kind of attorney ASAP. You should consider treading very carefully if this is going to the situation where a formal complaint is being filled. I know you feel terrible, and even though you may feel the need to apologize profusely and whatnot, sometimes you may actually incriminate yourself more. On the other hand, an apology is sometimes enough to quell the situation. Regardless, I think it would be a good idea to talk to some kind of legal representation, to see what the best manner to proceed would be. I would hate it if you got in trouble with a licensing board because of this mistake. I am surprised that the MD in the office is insisting a complaint be filed, because HIS/HER practice will come under the microscope, and I am certain no one wants that. I hope all works out the best for you, and this situation passes with the best possible outcome.

We had a office meeting they threw him under the bus, and told him either he stops his bs or he is gone also told staff they've got to start showing me the same respect and if they don't they will be dealt with at the end Of the meeting he threatned to resign

Well it has been awhile since I updated. I received a call from management last week and they want me to meet with a new md they are thinking of bringing on to replace the md who is there now. He is unaware of this ( so I have been told). This md will only be in the office 1-2 days a week and will essentially increase my work load which is fine , the only reason other than they have been wanting to get rid of them, is I am thinking that I am more cost effective from a financial standpoint. Hope I am not wrong.
