I am the Director of Nursing at a small(65 bed) assisted living. In the past this facility has always copied the POLST form and posted it in the residents room. In the closet, on the back of the door that kind of thing. I am curious how others assisted living places do it. We have it in their chart, its in our electronic charting on the computer also. I want to use the name plate next to the door on the back of the name just put a sticker that says DNR if they are DNR. Because we are their home I don't feel like placing this legal document in their room is a necessity. There is so much info on that form that a cna does not need. The only thing they need to know is if they are full code or DNR.
