I am in Geriatrics and see consumers in the home for a non-skilled Home Health agency.Our clients are sick enough for admission to a nursing home but the State allows money for care so they can stay in their homes. It is called the Waiver Program and this time Government is spending $ wisely.I see the personal care aides and consumers in their homes to make sure the Area Agency on Aging treatment authorization plans are OK.
However the Area Agency on Aging sometimes doesn't give me any diagnosis- so I see some people BLIND.
I have forms to fill out that ask me to list changes is client status, but how can I do that with no prior information of a base line.
I have never encountered seeing a client without a diagnosis @ the ready.
It makes no sense to authorize care, but not give any health information to assist in providing the best care.
I am required by the AAA to see the clients every 90 days, so they require my eyes , but then give no weight to the medical needs. BIZARRE!!
I called the STATE to talk about guidelines and the consultant said he had been in the field for 20 years and this was the first complaint. YIKES!
I experienced this lack of info in 3 counties with 3 different AAA's.
Does anyone else have a similar experience?
I trust my assessment skills , but our client's get tired and are often poor historians....why am I trying to re-invent the wheel when the DR. and AAA already have pages and pages of info????

Perhaps you can circumvent the system by dealing with your client's doctor directly. Just get yur client's permission and go for it. You sound like the victim of beaurocrats with their heads up their a**!
