Is it home health or private duty? Neither are good to start out in as a new nurse.

New grads should NEVER start with a trach/vent case in home health/private duty. Sounds like you are in over your head before you start

Take another job. If not possible, take another case.

Read everything you can get your hands on in the nursing literature about care of patients with conditions that are the same as AND similar to your patient's. Review emergency protocols for the employer, memorize them if need be. Get malpractice insurance if you don't already have it.

Also, read everything you can over at the Private Duty Nursing forum.
I don't suggest posting there, though, until you have more experience. New grads are not welcomed with open arms in the PDN forum.

Look for a vent certification class and take it asap.

Run. Any agency that puts you in a vent case as a new grad is using you're warm body and license. They will hire anyone (no offense). Don't lose your license making a mistake like this so early in your career. You're smart to question the case. This is an unsafe assignment.
Oh, and shifts are PDN, visits are home health. And as other people stated, new grads and PDN/home health don't mix. Good luck.
