New HH Nurse

Hello!! I am new at the HH specialty. I have been doing it about three weeks now and my most challenging part is the charting!! Help please!! I am used to charting by exception and all this lingo is killing me!! It is all done on paper. If you all know of any sites or ideas, programs, etc. please do share. I am stumped and tired of correcting my notes.

So far I adore all of my patients and have enjoyed this so much more than I thought I would. I am finally getting the oppurtunity to know and understand these individuals after years of just shoving medicine down their throats to get everything done by the end of the shift.

Each agency has their own requirements. I have worked places where we have to write the whole assessment in and also have used flow sheets with notes added as needed. Then there is the Oasis monster which is something in and of iteself. Just give yourself time to get used to it. I know of no programs, etc. to help you with this- just time to get used to it. Make sure you look at the 485 to know the goals, interventions, etc. on your patients so you can be sure you're covering them. Talk to your co-workers since they are a wealth of information and go through some of the charts to see how others chart.

Did the agency give you a copy of the COPs for the pay sources you serve? It takes some time to get used to the lingo. I know paper for paper based agency is very large. To bad point of service technology is so costly.

It is nice to know that it will take a while. I really was overwhelmed last week with all the notes I had to redo. Very frustrated. I am enjoying these patients and I hope that I will be able to pick up.
