I need tips on how a student nurse give good hand-off communication (end of shift report).

When I receive repot, I want to know when this person came, why, what have you do e, how the person is doing, any falls or pain, where's the family, any extra precautions like combative, heavy, wonders, anything that paints a picture of the pt.

changes... focus on their status changes from when you clocked in.

Start with a brief intro: age, dx, code status, then work your way from head to toe, kind of like giving a dictation of your head to toe assessment. Keep in mind to note if they are ambulatory, if they require assist, fall risk status, diet, iv sites and what they have running. Last things i state are any pertinent labs, if they are scheduled for procedures, and things that need to be endorsed to the next shift to be done. Some sites have handoff sheets, maybe you can see if your facility has some? Giving a good end of shift report comes with practice, you'll get better the more you do it.
