Need incident report- home health care - field nurse HELP plz

Hi I'm new nurse and got a job for field nurse home health where I go see patient to patient, one pt I had which was assigned to me had a pressure ulcer on buttock but due to her weight I didn't fully lift up to check fly next few days a nurse that went in say there was a ulcer and now my CM that she need a incident report? I try asking my CM if company had a form how do I do it cuz I'm fairly new and I feel so lost and CM seem busy all day ---small company only couple of CM...

Any advice of how I do it or what I need to do? I feel worry cuz I'm totally new to this and no one in comp I can ask since I'm new...any help appreciated...

If they don't provide a form, simply write a statement on plain paper telling all the necessary information. Facts only, no opinion. Can be written on the agency's form titled "Communication Note", which is only a blank form with lines and the company logo. Your supervisor will prompt you if she feels you have not addressed all required information.
