Need help writing a 3-part nursing diagnosis

I hope you don't mind the intrusion from a student, but when I need help, I go to the experts, and that's you all! I have to write a 3-part nursing diagnosis for an assignment but I'm having trouble knowing what to write. The scenario is:

"Mr. Jones is a 36-year-old African American veteran who has been homeless for the last six months. He has been admitted to the psychiatric acute care unit with a diagnosis of polysubstance abuse (including heavy use of ETOH, crack, and nicotine). He also has NIDDM, and appears very malnourished.

"He has been separated from his wife of 10 years for the last eight months. He is the father of two boys, ages four and six. Mrs. Jones has returned to work as a computer technician at a data processing company.

"Previous to his drug use, Mr. Jones owned a small electrical shop. On admission he indicated a desire to het his life together so that he can be rehabilitated and reunite with his wife and children. The wife says that she loves her husband and will do whatever she can to help with his treatment and rehabilitation.

"Since admission to the unit, Mr. Jones has reported that he has not been able to sleep at night. He has been visited by his minister and his wife, who reports that the children "are eager to have Daddy well and home."

I don't know how to write a 3-part nursing diagnosis for this. My teacher wants the format to be "[nursing diagnosis] r/t ________ as evidenced by _____." I asked my teacher for more context but she said all the necessary info was provided in the scenario.

I'd appreciate any ideas, thanks in advance! :)

You can actually come up with several nursing diagnosis from this scenario.

One would be, inadequate nutrition, related to substance abuse, secondary to being homeless, as evidenced by significant loss in weight (or appearing anorexic).

Another would be, risk for ineffective coping related to separation from wife and children, as evidenced by inability to sleep.
