Need help with nursing diagnosis for a postpartam patient please!

I need a main diagnosis to work up on my patient! She is 16 yr old G1P1000, gave birth lady partslly, with an epidural, 150ml blood loss, there was meconium, no gestational problems, no health problems except she is GBS +, no diabetes, htn, she doesn't abuse drugs or alcohol or smoke, no diseases vitals all within normal limits. I was thinking risk for fluid volume deficit r/t lady partsl birth but I'm not confident in that. I really need help with this!

How about pain/alteration in comfort? Or knowledge deficit: anything regarding to the care of her infant or care for herself. Because this is her first child I would say knowledge deficit might be a good one. You can use this as an opportunity to provide a lot of teaching. Good luck!

Pain would be good but I'm not allowed to do pain or constipation and it has to be a physical problem or need and knowledge deficit doesn't fit, but thanks a lot anyways!

Did she have an episiotomy or a perineal tear (Impaired Tissue Integrity)?

Is she Breastfeeding (Effective or Ineffective Breastfeeding)?
