Need help with filling out FBI fingerprinting cards

I'm not sure if this is the right place to ask, but I need some help with filling out FBI fingerprinting cards for the criminal background check for the North Dakota RN license by endorsement. Some of my questions are:

1. In the space in the front of card where it asks for employer's name and address, what if I'm not hired yet? I'm in the process of being hired by a hospital in ND but technically I'm not an employee yet.

2. What is SPBI # and do I need to fill that out or leave it blank?

3. In the back of card, it says "Use for applicant purposes only" and it asks for name of company or organization and citizenship status, etc. Do I need to fill out this page if I'm not hired yet?

4. I have my citizenship through naturalization, so should I fill it it?

5. About scars and marks, do moles count? (Stupid question, but I just want to make sure things are filled in properly so it won't be returned to me).

Look on your Board of Nursing website, it might have instructions that are specific for this purpose.

I checked the NDBON but it doesn't say anything about filling in the back of the card.

I did this at this police department for a nsg license with live scan which is like the pyxis scanner only the cards go in the machine.

The police officer called the state police fingerprint expert before he filled out the card. Take your ID passport, birth certificate social secirity drivers license and BON paper work. Dont write on the card at all. Let them do that.

These cards go to the FBI and North Dakota State police.

The police love to help nurses you know. It will be fine.

That's what I did. The person who did my fingerprints didn't fill out anything, he just signed his name and told me to fill out the info myself. I called NDBON today and asked about what else I needed to fill out. I did what she told me and mailed them out this afternoon. I figure if I did something wrong, I could always redo them. I have a temp permit to work in ND and have until the end of Nov. to figure it out. But I rather get everything done correctly and now so I don't have to redo them.

I think the officer who did my cards obtained the number from the expert at the State police. So it's good you left it blank. I hope it all goes through. I would call the BON in a few weeks to ask for an update on your application and just to have a contact person there. They are usually very pleasant and helpful.
