Hi all!
I was taught in school that documentation can not be delegated to UAPs however I have came across two questions on UWORLD that asked about UAP delegation and the correct answers were as follows:
"Measure urinary catheter drainage and document findings"
"Obtain oxygen saturation reading and record"
I understand that UAPs can measure output and obtain VS but for NCLEX purposes can they still do it if the answer says and document? (Again for nursing school these answers were considered wrong)
Second: I got a 70% on the assessment exam (UWORLD) which game me 95% likelihood of passing NCLEX first time BUT since then taking 75 question practice exams on UWORLD I have been getting in the 50s 😞 I take NCLEX in 4 days... this worrys me a lot

All the CNAs in the hospitals I've been in measure vitals and input that info into the computer charting software. It is within their scope of practice as long as THEY are the ones who measured vitals. If the RN measures vitals, she cannot delegate the CNA to document those vitals. Does that make sense?

Ditto on Nicuismylife. The concept is that you can't delegate your documentation to anyone. Including another RN.
If you did it, you document on it.

did anyone know the best way to do uworld please I need good way to study because I did the questions and read the rationale at the same moment is it good or bad doing that way I able to do only a few questions please anyone help I am going for my test in March I just started study

For the NCLEX I just used Uworld as my study guide. At first I did a couple of questions with all the categories selected so the system can get some data about my knowledge. From there I looked in the areas that I was struggling most and from there each day, I would do questions only from a certain category. For an example, on Monday I focused on Adult care, next day was fundamentals, then child life and so on. I made sure to read the rationales. I did at least 60 questions a night but not all at the same time though because I didn't want to overwhelm myself.

I just got my results and PASSED NCLEX!! UWORLD and the study guide found on here somewhere were the best preparation I can think of . My school did many different programs and those two was the most useful.
For UWORLD I did the 75 questions timed all in one sittting with ALL subjects and categories in the two weeks before NCLEX and acted like they were mock exams , telling myself I was going to get 75 questions that was my NCLEX goal
