Multi-Patient Assessment Sheets

Does anyone have assessment sheets that they use for multiple patients that I could look at? We're beginning to get at least 2 patients now -- which is great but I'm having problems coming up with a form that helps me organize the info on both patients. Please let me know what works for you as far as keeping all the info straight and organized ... thanks!

I'm not sure what you mean, but I write on the back of my patient info sheet. We are assigned to a nurse and take a choose a couple of her patients to care for each day. We get the printout on each patient in the morning and get report with the nurse. I just keep track on the patients printout. I write down each hour on the back of the printout and write in what needs to be done that hour. I cross it out as I do it so I don't forget anything. I throw the papers away in the shredder box when I leave.

I got a form from a nurse, what she used, then modified it to suit me and my own style quirks. It's for managing four patients on the same sheet. I have put it on its own thread, since I'm unable to attach here. Enjoy :)

OP, I used this form throughout school until last semester when we didn't have to do clinical paperwork. You are talking about having info that you can take home and do your reports with, right??

I thought she meant one page to keep track of multiple patients, so I put my form on another thread! I wonder if not being a Premium member prevents me from attaching to an existing thread...? Hey, taking away my paperclip privilege will only hurt the masses! I put up alot of good study tools.

You DO have good stuff, though...seems a shame you're having such a hard time posting it 

might be a membership thing, probably is. So, I'll post a new thread with the info when I need :)

I only post useful stuff for others, so it's not like it's benefitting ME any!
