Modifying Measuring tools

I am going to conduct a study for a quality improvement project and I do not want to develop my own measuring tool. Rather I wish to use a tool that is already tested and considered reliable and valid. However, I do not need to use the entire tool for my study. Thus my question, when using a measuring tool such as a questionnaire, is it possible to remove questions that are not needed for my study without the tool being considered a new tool?

If so, how would I note that in a paper? For example, would I state that the tool is the blah blah blah tool with the exception of two questions? Also, would the validity and reliability of the tool be altered if I remove some questions?


-Not in school at this time, but I am a future DNP or PhD student who is working hard to enhance my CV to be accepted into a reputable program. :)
