   Hey MJ3lasvegas, I found out that NEVADA BON doesn't SSN to process my application and sit in NCLEX exam. That's way better than in CO BON where it requires SSN before processing my application. Thanks a lot for you help! I'll apply soon in NEVADA.

   I already have ITIN # and they don't take that. They keep insisting that I must provide SSN in order to process my application. The form that you're talking about is the same SSN Affidavit form and is required you not be physically present in the US.
