  1. Brief handovers in ER

Thanks.Yep, I am familiar with those scenarios as am a 25 yr ER veteran but we are trying to implement better handovers, short but more structured to avoid errors and improve communication. Thanks for input.
  2. Brief handovers in ER

What type of handovers are people using in the ER to improve communication when going on breaks? I know SBAR and for out of department communication have used "Tickets to Ride". Anybody got anything else that works great. It needs to be simple and ...
  3. Efficient triage system????

Does any one have a superb efficient triage system that they want to share with me? We have a work group tomorrow to try and re-model our inefficient, slow process. I know that some nurses are not suited to triage and take 10 - 15minutes to do an i...
  4. Highest BP you have ever seen on a pt

That is why I left her as a cat I ( in the 5 system) I did not want to code her in triage! Thanks
  5. Highest BP you have ever seen on a pt

Funny this topic has come up .Within the past 10 days I had a patient present , who had been referred by her optometrists. He had seen a new bleed in her eye and she was hypertensive by his BP machine. When my machine would not work in triage and kept...
  6. What nursing skills do you use the most?

I believe that your assessment skills should be excellent, ensure that you listen to the patient and to the relatives.Believe them when they say that something is wrong, the relatives know that person better than you. Listen also to your own intuitio...

  10. 2 hour reassessments in Triage

We already have 2 triage nurses, a diagnostic nurse that starts labs , does EKG's and now we are using a reassessment nurse! Just a busy ER, you know how that can be! All those nurses and no computer terminals to use! Thanks for the input!
  11. 2 hour reassessments in Triage

Thanks for the input. Our 5 tiered system in CA must be the other way than in PA. A 1 is a CPR, a 5 is a prescription refill, so I think you mean that giving a narc bumps them to a 1? Possible a 2 or 3 for us though as no airway compromised, breath...
  12. 2 hour reassessments in Triage

We have just started having a nurse be the Reassessment Nurse assigned in Triage. We do use a 5 tiered system. But I was wondering if other ER's are doing the same. Are you reassessing the 4 & 5's every 2 hours or are you using a different time ...
