lvn/lpn vs nurse case manager

I am wondering what the major differences are between an lvn vs a nurse case manager? job duties and salary? thanks :)

I think it depends on what the setting is-- an Assisted Living Facility, Home Health, LTC? There can be a lot of variety in those job descriptions!
