Licensure Verification - Please help

Hello everyone ;);)

Could anyone help me with my query, i am a Registered Nurse in BON-NM and i am currently applying for Australian Health Practitioner Regulation Agency (AHPRA) for Nurses, and i need to fill up a licensure verification form from BON-NM so they can forward it to AHPRA. I found in BON-NM website a form however i am not really sure if this is it ill be using.

Is this the one i will be filling up with information (http://www.bon.state.nm.us/forms/lic...t_June08.pdf)?

I am just scared of sending it wrongly to the BON-NM..

Hope someone will enlighten me with this.
