Ok so i need help. I am doing my pediatric care plan on a patient with Lemierrs. I understand the patho and cause and complications and such but i am having a really hard time figuring out what nx dx to use and in what order. please help. these are my top 3, not including r/f and psychosocial. Should i also use infection? ineffective airway clearance? thank you!

Care plans are all about the patient assessment and what is important to the patient right now. Tell me about the patient.

((HUGS)) You need a pretty big pocket . Remember I have been a nurse a LONG time...it comes with experience. I don't know why schools have such trouble conveying simple ideas and seem to make it harder. It really is very simple. LOOK at your patient. What do they need? If this was your family member what would be important to you?

Nursing diagnosis is not a result of medical diagnosis. You cannot EVER say, "My patient has X medical diagnosis so what nursing diagnosis goes with that?" MAYBE you can say, "My patient has thus and such effect which tells me his nursing diagnosis is Y. He has these symptoms because he has X medical diagnosis..." sometimes, but not always.
