I am a new grad applying for jobs as an RN. My question is- is a cover letter required to go along with the resume?
If so, I am applying online and they only have one option to upload a resume- should I include the cover letter and resume in one word document???
Any suggestions will be helpful.

If you're applying online, don't do a cover letter. If any of them want to interview you, take a hard copy of the resume along with you and include a cover letter then.
If you apply on paper, include a cover letter.

I am applying online, and also giving a copy to the nurse manager where I am an extern. Should I address the cover letter to the nurse manager when I give it to her or the the recruiter. If I meet the nurse recruite I will address it to the nurse recruiter separately?

You address it to the person you interview with, or the one you submit your resume to. If you submit the resume to the recruiter, address it to her. You do not need another one if you later interview with the manager.
