Making a Care Plan

    Hi there,

    Can you be more specific? Do you just need a care/ teaching plan or Nursing Diagnosis>

    The way we do it, is to come up with a Nursing Diagnosis statement following NANDA's criteria and according to Maslow's Hierarchy of Needs. Our teaching plan is then made from our stated interventions & expected outcomes.

    Example:

    Maslow: need Oxygen

    Ex: Ineffective airway clearance related to chronic asthma as evidenced by client's SOB, audible wheezes in RUL, intercostal breathing and decreased O2 sat.

    1. Above is the Nursing Dx statement.
    2. We then need to come up with 3 Nursing Interventions with source documentation.
    3. For each of the 3 NI, we then need 3 Rationales with source documentation.
    4. Then one expected outcome per Nsg Dx
