Inhaler over dosing

I am a resident care manager at an Adult Family Home. I have an resident that uses an inhaler with inhaler spacer twice a day that is one puff. The resident has dementia with impulse issues. So we prime the chamber and hand it to the resident while standing by. But if we turn our back or get distracted by another resident she will add puffs. I have been looking for spacers that we could remove the inhaler but I cant find any such thing. If anyone has any ideas how to prevent this over dosing other then controlling the device physically the entire time it would help a ton.
