Hi!
Any help would be much appreciated. I am unsure about using the Nursing Diagnosis of "Impaired Social Interaction related to Anxiety." Is this correct or should I be using Ineffective Individual coping related to stressors greater than client's ability to manage?"
Nursing Diagnosis
Impaired Social Interaction related anxiety (exceeds ability to cope adaptively in social situations) as evidenced by client's report of tremors, perspiration, palpitations (anxiety symptoms), client's report that he doesn't feel "comfortable eating in front of other people," client verbalizes a fear of embarrassing or humiliating himself, inability to maintain relationships with friends (turns down dinner invitations, friends have stopped inviting client and his wife), excessive drinking of alcohol in social settings (bar with coworkers and friends).
Outcome
Client will verbalize and utilize three strategies to promote effective socialization with out the use of alcohol by time of discharge.
Nursing Interventions
Assess client's past coping skills.
Assess the client's current coping skills.
Help client to identify alternative courses of action.
Provide supportive individual and group therapy when indicated.
Encourage client to identify his support system.
Encourage the client to express his feelings about social interactions (work and social).
Discuss with client three new positive coping strategies to decrease feelings of anxiety in social environments (work and social). Encourage client to identify three ways he will maintain sobriety following discharge.
Teach client anxiety reducing strategies: deep breathing, meditation, use of exercise, yoga.

either one is fine. (and good job, too:yeah:)
you could even use both, and address both. or if not, ask him which is most important to him. it's always appropriate to validate your assessment and diagnosis c the patient.

Based on the data you wrote, I would say "impaired social interaction r/t fear of rejection or failure AEB physical manifestations of fear and avoidant behavior."
and I'd address the alcoholism with a different dx. I think what you have is really good, though. I'm in psych now, so I'm no expert yet :)
Hi! Thank you for the great feedback. The client in the case study has social phobia and fear of eating in public. He drinks in order to relax and quell his fears in public.
Maybe my nursing diagnosis should focus on "Impaired Social Interaction related to fear of rejection and embarrassment as evidenced by anxiety symptoms (tremors, perspiration, palpitations), avoidant behavior and defense mechanisms such as using alcohol in social environments. Any thoughts?
This assignment requires 1 nursing diagnosis, 1 outcome and 6-7 interventions. I would love to do more, but need to make sure the one I turn in is amazing!
