hello everyone im going to submit to eres the application form for my course by course evaluation there is a question there where i have to put the name of the state im applying for and the address of the bon, i just want to know which address should i put in it is it:

this is the address where all my documents for application should be sent according to the guide given to me.
lastly will eres give me the evalution resukt or send it to the state im applying for? since im doing the ces and toefl first then send the application to the state to take nclex

It needs to go to Continental Testing Services, there are the ones that handle the applications for the BON.
Much has been written on this very topic on the International Forum. Since you are just starting the application process for licensure, I am going to move this to the International Forum where there is more information that you need to be aware of.

And to your other questions: The credentials eval will be sent to the state for you as well as the TOEFL results provided that you request that they be sent to Illinois.
You may wish to consider the IELTS for your English exams since many from your country have issues with passing the speaking section with the TOEFL iBT. With the TOEFL, you have to speak into a machine and then your answers are recorded and sent on, with the IELTS, you actually speak one on one with a human examiner directly in front of you and many find this easier.
I also prefer the CES done by CGFNS as that is accepted by all states as well as most universities if you wish to continue on with your studies. If you decide to change states, then you may have to go thru the process of the credentials eval again to be able to get it accepted.
Suzzane4 i want to ask for your advice, what do you think i should do first the CES or TOEFL? I am currently waiting for some clear answers from the people who have done this before and they are really helpful in clearing the matter to me. I am just worried that the CES will give the result directly to intercontinental testing services and not to me since i have not yet submitted an application to them(the TTS). The CES is my first step do you think its right?
I chose TOEFL since it is the one required for Illinois but they also accept IELTS i think. Anyway thank you for letting me consider IELTS i did not know the difference before, I would rather talk to a personel one on one rather than a computer.

If you trained in a program that was conducted completely in English, then the language requirement is waived by Illinois BON. You will need the language exam for immigration and to get the Visa Screen Certificate.
The credentials eval is always going to be sent directly to the party that you have listed on the application that you place with them. It actually cannot be sent in by you.
CES should be your first step in the process. Just suggest that you do a search here for threads that are specific to getting licensed in Illinois, they will give you all of the steps that you need to follow. Much has been written on this topic here, that is one of the reasons that I moved your thread to this forum.
(Nice thing about using CES is that the same documents can be used for the Visa Screen Certificate afterwards, you will not need to submit everything else again to them, just pay the fee and submit the application for that. And pass the English exams......)

Healthcare Profession & Science course by course Report. Then the choices below the application says:

I personally would get the full course by course done as you can use it again if you decide to do further study.
Can you use the full course by course even if the BON of IL only ask for the "Healthcare Profession & Science Report"? I want to do further study someday.
Maybe something to ask the BON. Full course by course breaks down your transcripts a bit further than the Healthcare Profession & Science Report
