Let's say you were directly involved with getting a provider fired from your facility. You know HR proceedings are confidential, but you also know this person isn't an idiot and knows the scenario they got fired over and knows you were a primary player in that scenario.
Now your family member is a patient in a different hospital and there is that provider. You have signed a confidentiality agreement that you wouldn't say he or she was fired from your facility. But they have seen you with your family member and you sure as heck don't want that person involved in family member's care. How do you get administration at other facility to take your requests seriously without saying too much?

Simple "I'd like another provider." No need to provide a reason. Just be bland and matter of fact and don't get into any further conversation.

Is it your business to be so deeply involved in who cares for this family member?
If you are in a regular capacity, speak to the person about alternative providers.
If you speak for the person, ask for a change

Exactly this! You simply state you would like another provider.

Your family member has the right to refuse care from any provider without giving a reason. You certainly can insist that they refuse care from the provider you are concerned about without violating an NDA.
