How to put multiple schools in NCLEX app form for NY??

Hi! Before anything else, I gotta say that this is actually my first post in a nursing forum. I usually do stuff on my own but this little problem I have, I can't seem to find a good answer anywhere. Hopefully this would also be of help to those under the same situation as me. So here it goes..

I've been filling up my NCLEX application form for NY state when I came across ITEM NO. 17 (page 2 of nurse form 1) where I'm supposed to give an accurate record of my educational preparation (elementary, HS, and nursing college). Problem is, our family used to move around a lot then so I was forced to attend 3 different primary schools, and 2 high schools. Fortunately by the time I graduated HS, we've finally settled down and I got to finish my college degree in only one university :D

My question: how am I going to put all this information in the application form?

- is it necessary to enumerate each one or just the school/college where I completed elementary and HS?

-is the NYSED so strict that they'd make a big deal out of my pre-nursing college education even though I'm already a licensed nurse in my country and also with the CGFNS looking through my credentials (CVS).

Schools I've attended:

Grade 1 (RCCS) - 1996 to 1997

Grade 2-3 (MCHS) - 97 to 99

Grade 4 (MAM) - 99 to 00

Grade 5-6 (RCCS) - 00 to 02

1st to 2nd year HS (MHCS) - 02 to 04

3rd to 4th year HS (TDC) - 04 to 06

Sorry if I'm being too much of a perfectionist. LOL! I just don't wanna waste precious time in case they return my application just because of a simple avoidable mistake. Also my apologies if the same topic have already been posted. I've tried to search it up and came to nothing. Anyway, if u know of a site/link that can help, pls post it. I'd be really grateful :)

I would put the school where you completed that portion of your education. So list the primary school where you completed the final year, the high school name that is on your diploma and the college that is listed on your degree. You can also call the NY BON and ask how they would like you to word it and if they would like you to attach a seperate peice of paper detailing all of your education.

I would call the Board with any questions you have concerning the application.
