I'm just curious how this varies from hospital to hospital and by specialty.

I work on an ICU stepdown floor. Vitals are every 4 hours and I&Os every 8 hours.

Onco, Q4H vitals, Q8H I and O's unless postop, we do vitals Q2H x3 then Q4, and usually follow their urine (all pts) at least Q4, esp postop and chemos

tele/oncology; VSq4; I&O q12. Even on our downgraded patients.

total set of vitals q4, HR/RR/SpO2 q1 since they're on a monitor, BP q1 if they have an art line.

I work on a Med/Surg floor and vitals and I&O's are taken q4hrs.
(Now does that actually get done is another story, lol. We get a lot of patients who refuse vitals at 12am and 4am.)

total joint floor. fresh post op q 4hrs vitals for 24hrs then q 12. i&o everybody q 12

Vitals Q1, or more depending what drips we have them on, I&O varies, if they are stable then Q8. Stable tele-overflow patients get vitals q4 and I&O Q8. (MICU/SICU)

Unless we have orders to do them more frequently, in my ICU we do q1 hr vitals and I&O's and q4 hr assessments.
