I recently was hired by an agency to a hospital and I just wanted to know how do I add the agency or hospital correctly? It is confusing because I need to know if I just add the agencies name and the hospitals name or just the agency.

I have seen two ways:
FirstNurse Staffing at
Memorial Hospital of Smithville; or
Memorial Hospital of Smithville for
First Nurse Staffing

Resumes should be tailored for a specific employer so that should be considered. How many hospitals and agencies you work for also matter in simply fitting everything on your resume efficiently. For example, if you work for only one agency and 20 different hospitals (I've talked to such nurses in the LA area), you might want to say just the name of the agency and 20 different hospitals in some time frame. If you've worked at the same hospital for three different agencies, what do you think is best?
From an interviewing hospital manager perspective, she could care less about an agency name, only about the hospitals you've worked at. So personally, I prioritize that. I've been a traveler for 20 years and I can still fit all my hospital names on one page, but I've never bothered with agency names. Applying for a staff position once, HR made a fuss over agency names. Asking why, they replied it goes to loyalty!
Even more important than how you format your work history in the future is getting references now. They need to be written references from at least a charge nurse (higher up the food chain is better). Telephone numbers by themselves for references are next to worthless to future employers. A reference from an agency is similarly worthless. Think about it, the agency is just a conduit for money, they are not providing oversight of your work and indeed have little knowledge of it, that is done by the hospital manager.
Use a form that takes just a couple minutes to fill out, and get it (or several) ASAP on every per diem, staff, travel job you get and you will have a superior professional portfolio that will make you much more marketable. Such forms can be found online and Google images is a really good place to search, and PanTravelers also has editable reference forms and resume examples too.

I have worked several hospitals for the same agency so I list agency then hospitals underneath with the dates, manager, address and phone number
