have become very close to a patient who has asked me to help them die.

seeking assistance for this.

Please get direction from your supervisor. Your post could be interpreted a couple of ways:

1. You seek assistance to help this patient kill himself

2. Or (the one I hope) that you are asking for help dealing with the emotions of this patient's care.

Also remember in the UK if they are asking you to help them, you are opening yourself to a minefield and face prosecution. Definitely agree on speaking to your supervisor

I am thinking the real question here is , "why?" Is there fear of pain? Fear of loss of dignity? It seems the important thing is to work the problem, and pull in counseling help.
