horrible day( vent )

i had a horrible day at work yesterday. i was in charge of a 35 bed med/ surg / tele unit . i took one patient . he was in for a small PE , stable and up and walking and talking, he was just admitted to increase his INR. he was on lovenox and protonix and of course coumadin. i got so busy at the end of the night i forgot to give him his coumadin before i left at 1800... i remembered this afternoon i forgot to give it to him.... i feel so stupid i deserve a write up , his INR was 1.0 when i left. he did get his lovenox earlier that day.

at the end of the shift i had a patient that needed to be transferred to IMC and alot of families that distract me , plus 2 admits arrived on the floor and it was a mess....

i have made med errors over the 23 years of nursing , and i am hard on myself, because i want the best for my patients. here i am the charge nurse and i forgot to give my patient his coumadin.... :banghead:

Ohmygoodness, WHY are you beating yourself up?????? My goodness. You were unbelievably BUSY.

You are one of those nurses who expect yourself to be superhuman. Posts like yours actually bother me because it's like... so should I be beating myself up WAY more than I do?? I make at least one mistake almost weekly and I don't beat myself up quite like I used to. I don't make mistakes that are INCREDIBLY costly or hurtful, but you forgetting to give the Coumadin... someone most likely caught the error, did they not? You didn't mark it off the MAR before you gave it, did you?

Stop beating yourself up, please!! You are making the rest of us look bad! ;)

ooh you are so right, i do beat myself up and always have ... i don't know why , but i try to do it all ...believe me i drive myself crazy sometimes...

no i didn't mark it off i didn't sign for it ... i never even got close eonugh to the pixus to think about it.... i had so much going on , i forgot about it totally....

the charge that was coming on is very picky and i think i was nervous about all that was going on and forgot about the coumadin... i went in and saw the pt several times and he tok his shower and i made his bed and he was reading the paper in the chair by his bed .. he just didn't get his coumadin.... because after all that .. heck broke loose , and that coumadin went out of my mind...poof .... i didn' even get a chance to look at his mar again ...

i've been beating myself up for 23 years of nursing, and i'm getting pretty sore..lol..

Hey... I'm really not someone who should be telling people not to beat themselves up. I actually have a natural tendency to do it to myself. =) In all aspects of my life, I have a tendency to do it. Over the years however, I seem to be getting better about accepting the fact that, as a nurse, I'm gonna make mistakes! Especially when working in utter chaos!

Now, on those nights when we aren't busy and I still make mistakes... that's when I REALLY beat myself up! I'm actually someone who seems to work better and pay better attention when I'm put under pressure, though. =)

I think we all have had these days. I have been a nurse for 2 years and I recently went to work on a med surg floor after working home health since I graduated(more money but less skills). I have felt the same as you for the past few weeks. I started this new job and only got 1 day orientation. I feel overwhelmed at times wondering what I was thinking going from home health to med surg. I am hanging on but with a few tears at the end of a 12 hr. day.....:bluecry1: I am hoping it will get better. By the way I didn't even know about this website it is a good thing~!! We can all come here to vent and tell our stories....
