High BUN and Creatinine

I would like to know what is the highest bun and creatinine that you have seen. My previous highest was aprox 120/13, with the pt very confused.

My new highest is now 136/21.92 (I wrote it down to make sure I got it right). He was asymptomatic, and was still urinating. It took 3 trips to Hemo to get it down to my pryor highest. I'm sure renal nurses have seen it this high before, but I was curious as to how often. I used to think that greater then creatinine 15 was lethal. Also how many non-renal nurses have seen it.

I've never seen one that high working with med/surg patients.

I was working med surg and saw a BUN of 120, and creat of 14. Pt alert and oritented. Pt did recieve a few rounds of hemo and was down. Pt was SEVERELY dehydrated, causing the renal failure. Haven't seen one that high in a otherwise healthy person since.

I have seen many high numbers. dont forget to look for GI bleeds many things can make a BUN rise

I have a patient in the unit sitting on a BUN of 130 every day. Urinating fine. Creatinine is only in the 2's. No apparent mental status changes. Very interesting case.

Check the pt's H&H if the crit is low maybe bleeding!!!!!!

From memory his HG was aprox 6. He got 1 unit in the ER and 2 in Hemo. He went into renal failure due to chronic HTN and refusing to take his meds. I was sure it was a false reading, but the following post hemo readings mached up, and came down as it should. He was checked out, and we couldn't find any evedance of a bleed.

Not lethal but certainly the patient is in renal failure. Many patients on dialysis still produce some urine. 136 + 22 is high, never seen dehydration do this alone.

Today he's at 165. Platelets aren't working quite right anymore. But he's still mentating fine. So weird.

does anyone know the significance of creatinine in jp fluid after a cystectomy or nephroureterectomy?

BUN 171 CR 10 Ammonia 89

My mom has Multiple Myeloma, but she has been very healthy throughout her treatment. UNTIL thanksgiving. She had recently had a dose of ZOMETA (my dad thinks this med is the culprit of the following scenerio)... A few days after her ZOMETA dose. Her kidneys failed (Oncologost explained in laymens terms "gunked up"). By the time my dad convinced myself and her that something was drastically wrong (extreme lethargy only .. and slight confusion.... and took my mom (walking in upright against her will) to the ER... 11-23-08 Her BUN: 171, CR: 10 Ammonia 89.

Needless to say.. she got worse before she got better. 1 week on dialysis machine in MICU. 4 days later she BUN 60 and Cr 3.3 Ammonia 17
