Hi all! I have recently begun an LPN to RN transitions course before starting RN school in Janurary. I am in despirate need of help! This course is online and only 2wks long so there is a lot of info crammed together and right now it is a bit overwhelming. Anyway, what I need help with today is this....I am working on an assingnment about subjective/objective and primary/secondary data. For each example we have to list one of each choices. I am having trouble with ones like: Client's wife says he does not sleep well, not sure on either one. Another example is this; The client is coughing.
Any help and/or clarification on this would be greatly appriciated! Thanks in advance!

client's wife says he does not sleep well - this is subjective, secondary data.
the client is coughing - this is objective, primary data.
"objective: perceptible to persons other than the affected individual."
objective data is information collected by using the senses. information that can be seen (observation), felt (palpation), heard (auscultation) (percussion), or smelled. objective data can be called signs.
signs are objective and can be observed by the five senses. rashes or other visible skin changes, lumps or masses that can be felt, and heart and lung sounds that can be heard are all objective signs. x-rays, ekg readouts, and laboratory tests also provide objective information that can be used to understand a health condition.
"subjective: arising out of one's perception of one's own state."
subjective data is the information the patient or the caretaker tells the nurse during the nursing assessment. subjective data can be called symptoms.
symptoms are subjective and are experienced and reported by the individual being questioned. aches, pains, spasms, weakness, fatigue and feelings of irritability or anxiety are all subjective.
symptom analysis: location, quality, quantity or severity, timing, setting, factors that make it better or worse, and association manifestations.

Thank you sooo much! Those were the answers I was leaning toward but I just wanted to be sure! Now if I can just figure all these other assignments out.....

just a quickie here:
subjective: what they (the patient) Say (see the S and S.)
Objective: see the O, doesn't it look like an eye?? (say yes) what you (the nurse) can Observe, see, hear.
Things like that stick in my noggin better, hope it helps you.
