help with R/T of mental health

hello I am just writtin a paper and my nursing diagnosis reference manual is horrible with psychiatric health nursing diagnosis. so I have disturbed sensory perception R/T disruption in brain biochemistry. however im not sure that makes sence, I always strugle with R/T and would love help. I also wrote disturbed thought process R/T content, delusional thinking. but i feel this is an evidence by, any imput would be helpful.

Remember that the first step in the nursing process is assessment. Without any assessment data at all, it's tough to comment on your nursing diagnoses.

Just looking at the structure of your nursing diagnoses without any patient data, the first one starts off good. Just add onto the end of it "as evidenced by (one of the defining characteristics listed for this diagnosis)."

You are correct that the second nursing diagnosis is not correct. It should follow the same pattern as the first one. Disturbed thought process R/T (one of the related factors for this diagnosis) as evidenced by (one of the defining characteristics for this diagnosis).

If none of the related factors or none of the defining characteristics fit with your patient, it isn't a valid nursing diagnosis for the patient.
