Hello!
I am a surgical technologist student who is preparing for my lab practical final to move onto my clinicals.
My final case assignment is an open bowel resection. Could anyone please offer suggestions for the 2 mayo stand set ups that I need for this case?
Much thanks!

Are you asking what instruments you will need on each mayo stand ? One mayo stand will be for the abdominal part of the case (which will remain sterile throughout the procedure, some use the term 'clean' part of the case, however sterile & clean terms are not the same or interchangeable terms) the 2nd mayo will be set up for the rectal part of the procedure ( this is termed 'dirty',once the instruments are used on the rectum the instruments are contaminated because the rectum area and contents have bacteria) Now for instruments on the 1st mayo:hemostats, mayo clamps, allis, long right angles, babcock's,short & long metz scissors, short & long needledrivers, suture scissors, richardson retractor, bovie tip extender, pool suction tip, long & short debakey forceps, tonsil clamps & have at least 1 loaded with a silk tie,# 10 blade x2 (on short knife handle & 1 on long knife handle),also have surgeon's preference of autosuture device ready to go with reloads such as GIA stapler. Now the 2nd mayo:3-4 hemostats, straight mayo scissors short,7-8 debakey forcep,a couple of lap sponges,EEA stapler (if doing proximal anastomosis) with some ky jelly. Remember everything that comes in contact with the rectum & inside of the bowel is contaminated, you don't want feces inside your abdomen which causes peritonitis which can kill the patient. Also, the surgeon which performs the rectal portion of the case will either change gloves or re-scrub afterwards,so have extra gloves & gowns.Hope this helps.

hi there,
on the other hand it might not involve rectal stapling. bowel resection can be small bowel which does not need to have clean and sterile separate mayo tables. i mostly use only one mayo table and most of the extra items i open as i go along. eg. long bovie, staples because depending on the bowel you are resecting you might need thick or thin staples. if you open them already, then there are chances that you might waste the high price equipment that you may not use. i keep everything ready on the side so that the circulator can give me what i need. however, i set it up this way:
5 roberts or curved spencer wells (oone loaded with 2/0 tie)
5 dunhills
1 mayo straight for suture cutting
1 makindoe/metzenbum
1 nelson scissors
2 different sizes of long and short debakey forceps
1 gilles to start with
a b.p handle with 10 blade in a receiver
i also pin a pak of 2/0 and 0 ties with ball and socket towel clips on to the mayo cover
as the surgery progresses, i add or remove stuff such as purse string applicator if surgeon prefers or babcocks and sutures such as 2/0 vicryl or 3/0 pds for bowel.
hope this helps.
