I would really appreciate what my peers standard of practice is on giving IV magnesium without any serum level first. Thank-you, fellow nurses are the most important response to my question, to me. This is a general question, no situational clinical scenario.

The standard of practice for replacment of any electrolye to to make sure you have a current level. Of course, with open heart patients, we replace mag for the first 2 days post op, but we always have serum levels draw everyday to ensure we are not overloading the patient. You are totally correct in your concern.
